After you have made a payment for your reading, please enter your birth data and the birth data of significant others that may be relevant to your reading.
If you have already had a birth chart reading with me, you can skip entering your birth data, as I will have that on file already. You are welcome to enter the birth data of other people that may be needed for your reading.
Once both payment and this request form have been received, I will contact you via phone or email to schedule your appointment.
